Reservations

To realize the reservation we request you sends us beforehand:

1- Personal information, Center of Dialysis of origin and information of the same one.
2- Dates of the meetings that will be realized (since we possess  limited seats).
3- Shift of preference (which is subject to availability).

07:00 to 11:00 hs.
12:30 to 16:30 hs.
18:00 to 22:00 hs.

4- A telephonic number to be located for any consultation.

After confirmed yout seat will be needed:

- Summary of Clinical History (route Fax or E-mail).
- Viral scoreboards with less than one month of realized (Hepatitis B and Hepatitis C).
- HIV with less than one month of realized.
